Guidelines for Using Independent Academic Administrative Services

The Faculty of Pharmacy at USU provides a complete guide for independent academic administrative services for undergraduate students. The guide includes research application procedures, recommendation letters, and other supporting documents.

This announcement provides the procedures for using the Independent Academic Administrative Services for Undergraduate Pharmacy students (Regular/Special/International/Extension Programs) at the Faculty of Pharmacy, Universitas Sumatera Utara, as follows:

  1. Students may download the application files for thesis title, research, identification, and raw materials, with the following notes:
    ➤ Choose the first format if the thesis research is part of a lecturer’s research project.
    ➤ Choose the second format if the thesis research is conducted independently by the student.

  2. Table instructions:
    A. Thesis Research Application in Faculty of Pharmacy USU Laboratories — to be filled in only if the research is conducted within USU’s Faculty of Pharmacy. Delete if not applicable.
    B. Thesis Research Application Outside Faculty of Pharmacy USU — to be filled in only if the research is conducted outside the faculty. Delete if not applicable.
    C. Application for Sample Identification/Analysis — to be filled in only if samples will be identified/analyzed at Herbarium Medanense FMIPA USU, LIPI, PPKS, or similar institutions. Delete if not applicable.
    D. Application for Raw Material/Drug Ingredient Purchase — to be filled in only if purchasing from institutions/companies that require an official letter. If direct purchase is possible, this table may be deleted.

  3. If a student needs an official letter for a purpose not listed in the provided formats, they may add it as needed.

  4. If the research title changes, the student must download the Request for Research Title Change form.

  5. If an additional supervisor is needed, download the Additional Supervisor Recommendation Form.

  6. If the research involves use of the animal house, download the Animal House Usage Request form.

  7. If Ethical Clearance is required, download the Ethical Clearance Request form.

  8. For research conducted at public health centers (Puskesmas) or pharmacies, download the Official Letter Request to Health Department.

  9. After all forms are completed, students must also fill in the official letter templates provided:

    • Research Letter for Lab at FF USU

    • Research Letter for Lab Outside FF USU

    • Identification Letter to LIPI

    • Identification Letter to Herbarium Medanense

    • Raw Material Purchase Letter

    • Animal House Usage Letter at FF USU

    • Animal House Usage Letter Outside FF USU

    • Ethical Clearance Letter

    • Letter to Health Department

    • Research/Survey Letter to Hospital

➤ All official letters must be printed in two copies. The date on the letter should be exactly one day after the request letter is submitted to the Academic Division of the Faculty of Pharmacy USU.
Example: If submitted on July 2, 2018, then the printed letter must be dated July 3, 2018.
