Description

The Faculty of Pharmacy at USU is accepting new students for the Pharmacist Professional Program for the Even Semester of 2022/2023. Registration is open to USU Pharmacy graduates from the class of 2020 onward.

Requirements:

  • Applicants must be alumni of the USU Undergraduate Pharmacy Program, graduating from 2020 onward.

  • Attach legalized copies of diploma and academic transcript (minimum GPA 2.75), 1 copy each.

  • Provide two (2) passport photos sized 3x4 cm and 4x6 cm with a red background.

  • Submit one IDR 10,000 tax stamp (materai).

  • Collect the PSPA registration form from Mr. Arfiansyah at the Finance Division, Faculty of Pharmacy, USU.

  • Submit the registration documents (in a green folder) and collect the exam number from Mr. Abdul Qodir Sihotang at the Academic Division, Faculty of Pharmacy, USU.

Additional Notes:

  • Pharmacy graduates who have not yet received their diploma and transcript may submit an SKTL (Certificate of Graduation) and Academic Record with a minimum GPA of 2.75, along with photocopies of:

    • Form C (receipt for the lux edition certificate).

    • If Form C is not yet available, a statement letter from the Academic Division must be submitted to Mr. Abdul Qodir Sihotang.

    • E-Journal.

    • High School Diploma.
